Pulmonary Rehabilitation
Supporting improved management of chronic respiratory disease in the community. The Pulmonary Rehabilitation Program consists of a rolling eight week program providing health and well being education and supervised rehabilitation exercise. There is also opportunity to attend a maintenance program and/or self management programs.
The clients are assessed and treated by a multidisciplinary team including Rehabilitation Consultants, Physiotherapists, Occupational Therapists, Social Workers, Speech Pathologists, Psychologists, Dieticians and Nurses as required and attend the group twice weekly.
Exercise Maintenance Program is run by an Exercise Physiologist in Fitness Centres. A ten week exercise program will be offered to suitable clients on completion of the Rehabilitation Program.
Eligibility
- Diagnosis of COPD residing in the Southern Health area
- Clients who do not live in the area but frequently use Southern Health respiratory services of emergency departments may also be eligible.
- The service sees clients with all stages of COPD - from mild to severe.
Cost
- Cranbourne and Pakenham sites:
Concession card holders is $50 for program or $3.50 per session (maintenance program is $3.50 per session) - Clayton site:
Cost is $50 for the program. There is no ongoing cost for the maintenance program at this site. - Springvale site:
Pulmonary Rehabilitation: The cost is $50 for the program or $10 per week up to $50.
The Exercise Maintenance Program has a small fee for use of the Fitness Centre.
Referral
Referrals must be in writing. Urgent referrals are to be forwarded by phone or by fax (please indicate urgency). Southern Health supports the use of the State Wide Referral Forms.
There is a preference to use the first three pages of the SCTT tool including the signed consent form. All referrals must include:
- Name, age and date of birth
- Address and contact telephone numbers
- Specific clinical problems requiring consultation
- Preferred language, please indicate if an interpreter is needed
- Copy of results of relevant investigations (Chest x-ray, CT scan of chest, lung function tests and blood gases, oxygen assessments performed within the previous 12 months).
