Shared Care and Discharge Coordinators (formally GP MHLOs)
These clinicians liaise between mental health program case managers and GPs to coordinate the shared care of patients experiencing severe mental health disorders. When these patients are ready to be discharged to the care of their GP, these clinicians ensure that this is done in a timely and efficient manner, in consultation with the GP, patient and their carer/s.

Mental Health Nurse Incentive Partnership Project (MHNIPP)
The Southern Health Mental Health Program is proud to be associated with the Dandenong Casey GP Association, the Bayside GP Network and the Greater Monash GP Network in this initiative that sees experienced Southern Health Mental Health Nurses working within GP clinics. These nurses assist in the monitoring of symptoms and the development of mental health care plans. This service is provided at no cost to the GP and patients.

Mental Health Promotion Program
The Mental Health program supports community agencies and initiatives that promote the mental wellness of the wider community. Staff are available to provide advice in this area or participate in Mental Health promotion activities.

Psychiatry Registrar Placement Program
This is an innovative program where Psychiatric Registrars are attached to GP clinics for the duration of their training. The Registrar visits the GP clinic once a month experiencing the delivery of mental health care within the primary care sector, while at the some time building valuable relationships with GPs in the community. This service is provided at no cost to the GP and patient.
Psychiatry registrar placement program
