Elective surgery
Southern Health offers a community based pre-admission service to all elective surgery patients who are waiting for surgery at Monash Medical Centre Clayton or Moorabbin and Dandenong Hospital. After seeing the surgeon, and once the pending surgery is confirmed, the patient is given a health status questionnaire and also a pre-anaesthetic examination form for completion and should be returned to the elective booking office. Patients are then directed into an appropriate stream of pre-operative care according to their questionnaire responses. All patients are assessed based on their response to the health status questionnaire. Nursing and medical staff perform the triage of the questionnaire with action taken that is appropriate to the responses given. Pre-admission assessments are available for download below.
