Home and community care - Community health nurses

Community and home care health nursing involves assessment, planning, evaluation, referral, advocacy and short term case management.

Eligibility

• as per home and community care guidelines
• frail aged
• disabled persons and their carers
• clients living in supported residential services may be eligible eg. planned activity groups (not nursing homes or hostels)

Waiting list

• information and appointment service to advise

Referral

• GP
• health professionals
• GP and health professionals must complete the service coordination tools template (SCTT)
• first appointment is usually conducted in the client’s home
