Elective surgery admission process
Your doctor will complete for you an Elective Admission form. On confirmation of your booking, our booking office will send you a Health Status Questionnaire to complete and return to our booking office.
Upon admission to the hospital please complete and sign the necessary formal admission form. You will be asked to provide a few personal details, such as contact telephone numbers, Medicare number and your doctor’s name.
