Accessing information about me

Southern Health is required to collect and handle health information and to ensure the privacy of your health information in accordance with the Health Records Act. The Health Records Act and Freedom of Information Act give you an enforceable right of access to your health record.
 

Application Process

The Freedom of Information application form outlines the information Southern Health requires to process your application.

All requests must be accompanied by a $24.40 application fee. Those holding a pensioner or current health care card must submit a copy thereof for fees to be waived.

All requests must include full name of patient, date of birth or date of death.


Further information

All requests are processed in accordance with the Freedom of Information Act, 1982. Such requests may include:
  • Patients requesting to view their medical record
  • Patients requesting copies of their medical record
  • Patients requesting medical information from their medical record
  • Requests relating to adoption information
Southern Health can decline to give you access to the whole, or part of, your medical record if we believe, on reasonable grounds, that to do so would pose a serious threat to the life or health of any person (including your own) and can decline to give you access to information about you given to us in confidence by another person, such as a friend or relative.
