When attending for an appointment it is important that you bring:- Medicare/Pension/Veterans Affairs card, Safety Net Number (if applicable)
- Traffic Accident/ Workcare Claim number or details (if applicable)
- List of current medications and your family doctor’s details
- Blood glucose meter, record book and insulin device (if applicable)
- Relevant scans/xrays/blood test results
- Referral document
