What are Community Aged Care Packages?
Community Aged Care Packages (CACPs) are packages of care that help elderly people remain living at home.
Bunurong Community Care provides CACPs to people who live in these areas:
- Bayside
- Casey
- Cardinia
- Greater Dandenong
- Kingston
What kind of help is available?
Some of the services available include:
- Assistance with bathing, showering or personal hygiene
- Nursing
- Social support
- Aids and equipment
- Transport
- Laundry
- Meal preparation
- Continence pads
Getting the right type of care
The care provided is based on your needs.
You will have a case manager.
He or she will help choose the right services for you.
An Aged Care Assessment will be required. This assessment helps us to work out which services are best for you. The assessment is free.
Your case manager will discuss all of the options for care with you. You, your family or anyone else representing you may ask questions or suggest changes at any time. Together, you will agree on the right type of care.
Once you have agreed on the type of care to be provided, a care plan will be written.
You will receive a copy of the care plan. This will explain the services you will receive.
Your case manager will make sure that the services are working well for you over time.
Is there a fee?
There are fees for the services provided by CACPs. The fees vary according to your circumstances. No one will be denied a service they need because they are unable to pay the fees.
Your case manager will discuss the fees with you.
Hours
Our normal business hours are Monday to Friday, 8.30 am to 5.00 pm. An after hours service is available to clients at other times for emergencies.
